HMIS Assist by FormSetu Privacy Policy

Information We Collect

HMIS Assist by FormSetu may collect the user's Chrome account email address for account identification and access status verification. We may also store plan, subscription, payment/license status, and support contact information required to provide access to the extension.

HMIS Assist by FormSetu does not collect HMIS login credentials, passwords, HMIS form data, health records, personal communications, browsing history, keystrokes, or precise location data.

How We Use Information

We use information only to identify the user, verify free or paid access status, manage subscription validity, process support requests, prevent misuse, and improve service reliability.

Payment Data

Payments may be processed through Razorpay. We do not store complete card, UPI, or banking credentials on our systems.

Data Sharing

We do not sell user data. Limited information may be processed by service providers such as payment processors, hosting providers, and backend services only as needed to operate HMIS Assist by FormSetu.

Official Status

HMIS Assist by FormSetu is not an official Government of India application. It is an independent helper tool made only to assist data entry on the HMIS portal.
