RCH Excel Entry by FormSetu is an independent third-party productivity tool. It is not created, operated, authorised, sponsored, or endorsed by the Ministry of Health and Family Welfare, National Health Mission, Government of India, or the RCH portal.
User verification is required
The extension assists with data entry but does not certify that source data or submitted records are correct. The authorised user must verify every record, selection, date, test field, generated RCH number, and submission result.
No medical or official advice
The extension does not provide medical, legal, administrative, or government advice. Labels and workflow assistance are provided only to support data entry and must not be used for clinical decisions or as a substitute for official guidance.
Portal compatibility
The RCH portal may change without notice. Such changes can cause incomplete entry, unexpected behaviour, or temporary loss of compatibility. Users should stop automation and review the portal whenever an unexpected result appears.
Authorised data handling
Users must have lawful authority to access the RCH portal and process the personal or health-related data they upload. FormSetu is not responsible for unauthorised access, unsuitable source files, incorrect user configuration, or failure to follow official procedures.
